The following documents are necessary:

  • A valid identity and address proof issued by a government authority.
  • A valid email address and phone number.
  • A declaration of the food safety management plan.
  • Proof of possession of the location/premises.
  • A no-objection certificate from the local Municipal Corporations or local bodies.
  • A kitchen layout plan.
  • A water testing report provided by an ISI-certified organization.
  • A list of food categories.
  • Medical certificates of employees.
  • A list of equipment to be used.

  • To ensure that your application is processed, print out the completed form and attach all supporting documents.
  • Submit the printout to the Regional/State Authority within 15 days of making the online submission.
